The recto consists of two pieces: a square card and a circular spinner wheel. The text across the top of the card reads “Columbus Buggy Co. Columbus, Ohio.” in all caps. Below is the spinner wheel. The corners on the outside of the spinner wheel have black and white text. The wheel itself reads “The Fortune Teller, Learn Your Future” in the center of the wheel in all caps. The top depicts a female fortune-teller showing a card to a gentleman and a woman looking at the viewer. The bottom of the wheel has directions for using it.
The verso is an advertisement for the Columbus Buggy Co., specifically, name plates for vehicles. The text “804 & 806 Walnut St., Kansas City, MO” is stamped at the top in red ink.
