This round silver medallion is suspended by blue-green ribbon from a bronze-colored metal bar pin. The bronze bar shows an antelope leaping over terrain.

Side one depicts a human figure with wings, raising a laurel leaf wreath with his proper right hand. The figure is mid-stride in front of a setting sun with a boat on the sea. It reads: "to commemorate the union of South Africa 31st May 1910"

Side two depicts four small circles, each with a design and the name of a portion of the country. "Transvaal" is represented with a shield and a truck or train. "Cape Colony" is represented with a heraldic lion. "Orange Free State" is represented with a tree. "Natal" is represented with two leaping animals. The four circles are enclosed with a rope motif around the whole. The text outside the rope reads: "ter kerdenking van der vereeniging zuid Afrika."
