The round copper colored medal has an image of the capitol building on one side. It is inscribed: "CAPITOL OF ARIZONA AT PHOENIX."

The other side has an image of a deer, a cactus, four trees, and three mountains. It says "DIDAT DEUS" under the image. It is inscribed in two bands around the image. The outer band says: "DEDICATION SOUVENIR FEBRUARY 25, 1901." The inner band says: "SEAL OF THE TERRITORY OF ARIZONA 1863."
